Bleeding complications associated with cardiopulmonary bypass.

چکیده
Bleeding after CPB has been difficult to characterize and its treatment equally difficult to standardize. The complexity of this problem is related to the hemostatic process, the technical variations in the operative procedures, and the many uncontrolled variables associated with CPB, including the effects of anesthetic or pharmacologic agents, the nature of the priming solution, hemodilution, hypothermia, the type of oxygenator, and the use of transfused blood products. Although there are multiple and generally predictable complex changes in the hemostatic mechanism during CPB, the temporary loss of platelet function is the most common and clinically relevant. This transient platelet dysfunction occurs in all patients undergoing CPB; however, it only causes excessive bleeding in a small percentage of patients. Unfortunately, it has not yet been possible to predict which patients will develop hemorrhagic complications, although prolonged pump times are a contributing risk factor. Over the past decade there has been extensive investigation into the management of bleeding associated with CPB, provoked primarily by the increased awareness of transfusion-transmitted viral diseases and the inappropriately excessive use of homologous blood products. Several approaches to autotransfusion of shed blood and autologus blood donation have been developed to minimize perioperative homologous blood transfusion. Pharmacologic agents such as desmopressin, aprotinin, and topical fibrin glues have also been introduced to improve hemostasis during CPB. The protease inhibitor aprotinin is particularly promising in the reduction of bleeding associated with CPB when given prophylactically. Aprotinin may provide new insights into the mechanism of CPB-induced platelet dysfunction. Desmopressin is indicated only for the treatment of bleeding after CPB. The management of bleeding associated with CPB will undoubtedly

Mechanical Bleeding Complications During Heart Surgery
Potential for mechanical bleeding complications is present in all forms of heart surgery, both closed and open. Successful management of such complications often is accompanied best by the rapid institution, or re-institution, of cardiopulmonary bypass, and in some instances it cannot be accomplished without the use of cardiopulmonary bypass. Therefore, the extracorporeal perfusion technologist...
متن کاملUse of recombinant activated factor VII for bleeding following operations requiring cardiopulmonary bypass.
Postoperative bleeding is a common complication following cardiothoracic surgical procedures requiring cardiopulmonary bypass (CPB). Serious bleeding complications requiring the administration of blood products, hemostatic drugs, and even repeat surgery are associated with considerable morbidity, mortality, and resource consumption. Therapy with recombinant activated factor VII (rFVIIa) may be ...
متن کاملA comparison of laboratory findings in coronary artery bypass surgery with and without cardiopulmonary bypass
Background : Quests for doing coronary artery bypass surgery by a technique with lower complications is going on, for this aim many studies compared patients undergoing CABG with or without cardiopulmonary bypass. This study was carried out to compare laboratory findings after coronary artery bypass in these two groups of patients. Materials and Methods: In a retrospective study, 167 patients ...
متن کاملCoagulopathy after cardiopulmonary bypass in Jehovah's Witness patients: management of two cases using fractionated components and factor VIIa.
BACKGROUND Changes in the Jehovah's Witness (JW) blood refusal policy now give members the personal choice to accept certain processed fractions of blood, such as factor concentrates and cryoprecipitate. METHODS Two JW patients undergoing complex aortic surgery who developed severe microvascular bleeding after prolonged use of cardiopulmonary bypass were treated with recombinant activated fac...
متن کاملComparison of the Effect of Voluven and Ringer Solution in Cardiopulmonary Bypass on the Bleeding After the Surgery and Renal Function in Coronary Artery Bypass Graft Surgeries
Background: This study compared the effect of Voluven and Ringer solution in cardiopulmonary bypass on the bleeding after the surgery and renal function in coronary artery bypass graft surgeries. Methods: Two types of prime solution including ringer - as Crystalloid- and Voluven were used in two groups and compared in terms of bleeding and renal function during the first 24 hours after the surg...
